ABSTRACT

Los dientes supernumerarios y la fusión dental son anomalías del desarrollo dental cuyas causas aún no se han dilucidado con certeza. El cuarto molar inferior, también denominado distomolar, es uno de los dientes supernumerarios con menor frecuencia de aparición clínica y su fusión con el tercer molar es una condición todavía menos común. A continuación, se reportan los casos clínicos de tres pacientes masculinos que presentaron fusión del tercer molar inferior derecho con un distomolar tratados mediante odontectomía (AU)


Supernumerary teeth and dental fusion are abnormalities of dental development whose causes have not yet been elucidated with certainty. The lower fourth molar, also called distomolar, is one of the supernumerary teeth with the least frequency of clinical appearance and its fusion with the third molar is an even less common condition. Next, the clinical cases of three male patients who presented fusion of the right lower third molar with a distomolar treated by odontectomy are reported (AU)

INTRODUCTION: The objective of this clinical case report is to describe the presence of bilateral fourth molars. The occurrence of supernumerary teeth (ST) is a relatively unusual dental anomaly. It is even more unusual to find patients with distomolar teeth also denominated fourth molar teeth. PRESENTATION OF THE CASE: This article describes a clinical case of a 24-year-old patient presenting with a maxillary fourth molar, who was diagnosed by dental x-ray, and surgical resolution of the case. CONCLUSION: Tooth extraction surgery was the clinical procedure chosen to treat the impacted tooth.


INTRODUÇÃO: O objetivo deste relato de caso clínico é descrever a presença de quarto molares bilaterais. A ocorrência de dentes supranumerários (ST) é uma anomalia dentária relativamente incomum. É ainda mais incomum encontrar pacientes com dentes distomolares também denominados dentes do quarto molar. APRESENTAÇÃO DO CASO: Este artigo descreve um caso clínico de um paciente de 24 anos que apresentava quarto molar superior, diagnosticado por radiografia dentária, e resolução cirúrgica do caso. CONCLUSÃO: A cirurgia de extração dentária foi o procedimento clínico escolhido para tratar o dente impactado.

Introducción: El término kissing molars se traduce como «dientes besándose¼; superfi cies oclusales de dos molares retenidos están en contacto una con otra en espacio folicular único y raíces en dirección contraria. Literatura científi ca ha reportado 44 casos. Cuartos molares o distomolares, supernumerarios que se forman distalmente a terceros molares; tamaño y forma variable, generalmente retenidos. Caso clínico: Femenino de 32 años con molestias en zona retromolar inferior izquierda, limitación de apertura, trismus, disfagia. En ortopantomografía, se observa cuarto molar retenido junto con tercer molar, superfi cies oclusales de ambos órganos dentarios retenidos en íntimo contacto dentro de un mismo saco folicular, raíces en dirección opuesta. Discusión: Existe controversia acerca de diferencia entre molares retenidos y KM, algunos autores sugieren que la ausencia de contacto entre dos molares retenidos no permite incluirlos en esta condición. Etiología aún incierta, algunos lo consideran como un evento aislado. Existe una teoría en relación con la formación de quiste dentígero; resorción ósea causada por presencia y expansión de éste, resulta en pérdida de hueso en zona mesial de molares retenidos facilitando movilización, inclinación y contacto uno con otro (AU)


Introduction: The term kissing molars is used when the occlusal surfaces of two retained molars are in intimate contact inside the same follicular sack and roots in opposite direction. Literature has reported 44 cases. Fourth molars, also known as distomolars, are a supernumerary tooth that grows distally to third molars; size and shape are variable, generally retained. Case report: Female 32 years old, pain in the left inferior retromolar zone, trismus, dysphagia. Fourth and third retained molars, with occlusal surfaces in contact, inside the same follicular sack, roots in opposite direction are observed in orthopantomography. Discussion: There is controversy about the diff erence between retained molars and KM, some authors suggest that the absence of contact between two retained molars, does not allow to include them in this condition. Etiology still unknown. Dentigerous cyst formation theory is proposed due to the bone resorption in the mesial zone of retained molars in this entity, which facilitates mobilization and contact between molars (AU)

Resumen: El cuarto molar, también llamado distomolar, es una variante de número y forma que se presenta como una alteración durante la odontogénesis, pueden ser eumórfico o dismórfico, único o múltiple, erupcionado o retenido, de forma unilateral o bilateral y se puede presentar en ambos maxilares. La etiología es desconocida, pero existen varias teorías que justifican el desarrollo de esta alteración dentaria como la duplicación de la lámina dental, su proliferación horizontal o hiperactividad de ésta; otros factores pueden ser los hereditarios, la división completa del germen dentario o regresión filogenética. El cuarto molar se encuentra en un espacio muy importante, que al desconocer su existencia o técnica adecuada para su extracción, las posibilidades de tener severas complicaciones transy postoperatorias son altas. Se realiza una revisión bibliográfica de diferentes artículos y textos en diferentes años para determinar de forma más clara el origen y formación de este diente supernumerario.


Abstract: Fourth molars, also called distomolars, are a variant in shape and number appearing as alterations during odontogenesis. They can be eumorphic or dismorphic, single or multiple, erupted or impacted, unilateral or bilateral, and can appear in both jaws. They are of unknown etiology, but there are several theories to justify these tooth alterations such as dental lamina duplication, its horizontal proliferation or its hyperactivity. Other factors can be hereditary factors, full division of tooth bud or phylogenetic regression. Fourth molars are found in a very important space, and when their own existence or the suitable extraction techniques are unknown, severe complications can frequently occur during and after surgery. A literature review was undertaken, examining different articles and texts of different years, in order to better determine the origins and formation of this supernumerary tooth.

Objective:To report one case of bilateral maxillary fourth molars,and to explore the occurrence and treatment method of additional fourth molar.Methods:The case materials of one patient with bilateral maxillary four molars were collected,and the information was record.Combined with reviewing the relevant literatures,the clinical data,complication and treatment of one patient with bilateral maxillary fourth molars were retrospectively analyzed.Results:The patient was diagnosed due to left maxillary posterior teeth area intermittent pain for several months and the increased pain for 5 d;at the same time,there was no abnormal appearance inducing the symptoms detected by speciality check-up.The pantomography showed the four molars in bilateral maxillary,and the fourth molars impacted the third molar root's absorption.The fourth molar crown was smaller and the root was shorter. The 18,28,29 teeth were removed,and the left maxillary posterior teeth area intermittent pain disppeared. Conclusion:The occurrence of additional fourth molar is a relatively rare demal dysplasia.However,there are some complications following with the fourth molar.Early diagnosis and treatment are essential.

Los dientes supernumerarios en la región del tercer molar inferior sonpoco frecuentes. Cuando se presentan en posición distal se les denominadistomolares o distodens. Algunos autores los denominan cuarto molar; ésta denominación es la menos aceptada, ya que el cuarto molarno forma parte de la fórmula dental permanente, sino que es un dientesupernumerario. Cuando éste se presenta paralelo a los molares, en lacara externa o interna de la mandíbula, se denomina paramolar. El manejo quirúrgico suele ser complejo y requiere consideraciones especiales. Presentamos dos casos de dientes supernumerarios en la región del tercer molar: distomolar y paramolar; describimos su diagnóstico, etiología y la técnica quirúrgica empleada.


Supernumerary teeth in the lower third-molar region are rare. When these are located in the region posterior to the third molar, they are known as ®distomolars¼ or ®distodens¼. Some authors refer to them as ®fourth molars¼, a term that is somewhat less accepted due to the fact that the fourth molar is not part of the permanent dentition, but rather a supernumerary tooth. When a distomolar occurs parallel to the molars, either on the outer or inner surface of the jaw, it is known as a ®paramolar¼. Surgical management is often complex and requires special considerations. We report two cases of supernumerary teeth in the third-molar region, one distomolar and one paramolar, and describe their diagnosis, etiology, and the surgical technique employed.

ABSTRACT

Introduction: The abnormal development of one or more teeth exceeding the number considered normal is defined as hyperdontia, or supernumerary teeth. Supernumerary teeth are usually diagnosed by routine radiographs, because in most cases they are asymptomatic and do not erupt. The prevalence of supernumerary teeth is low, ranging between 0.1 and 3.8%. Objective: To report a case of multiple non-erupted fourth molars. Case report: Patient M.N.S, 16 years old, female, leucoderma, in good health status attended the dental clinic referred by the orthodontist. On panoramic radiography, the germs of the teeth #18, #28, #38 and #48 were detected and retained. Also, the presence of three forth molars distally to the teeth #18, #28 and #38 were observed. The surgical removal of all third and fourth molars was performed under local anesthesia, in two steps: left and right sides. The patient has been followed for 1 year. Fourth molar prevalence is higher in males (2:1), differing from the patient of this case report. A few cases of multiple supernumerary teeth in non-syndromic patients are reported in the literature. Conclusion: Both early diagnosis and treatment are important to prevent crowding, failure of eruption, displacement of teeth, impairment of orthodontic treatment, gingivitis, and formation of abscess, cysts, and odontogenic tumors.

ABSTRACT

Supernumerary teeth are defined as the existence of a higher number of teeth in relation to normal dental formula. The development of supernumerary teeth can happen at different times of the life and can be detected either, clinically or radiographically. The relation between supernumerary teeth and nonsyndromic cases have been reported once in a while in the literature. In this report are presented four cases of nonsyndromic patients with fourth molars in the maxilla, which were described macroscopically and radiographically.


Los dientes supernumerarios son definidos como la existencia de un mayor número de dientes en relación a la fórmula dental normal. El desarrollo de los dientes supernumerarios puede ocurrir en diferentes momentos de la vida y puede ser detectada, clínica o radiográficamente. La relación entre los dientes supernumerarios y los casos no sindrómicos han sido reportados de vez en cuando en la literatura. En este reporte se presentan cuatro casos de pacientes no sindrómicos con cuartos molares en la maxila, los cuales fueron descritos macroscópicamente y radiológicamente.

OBJECTIVES: To present a rare case of mandibular fourth molar. METHOD: Brief review of the literature; presentation of the clinical data of the patient and images of the case. DISCUSSION: Supernumerary fourth molars are rare anomalies of the maxillofacial complex that are more common in the maxilla than in the mandible. This article reports a case of an impacted rudimentary type of supernumerary fourth molar in the mandibular arch.


OBJETIVOS: Apresentar um caso raro de quarto molar mandibular. MÉTODO: Sucinta revisão da literatura e apresentação dos dados clínicos do paciente e imagem do caso. DISCUSSÃO: quartos molares supranumerários são anomalias de número mais comum na maxila. Este artigo relata um caso de quarto molar de formato rudimentar, situado na mandíbula.

ABSTRACT

The occurrence of multiple supernumerary teeth in individuals without any associated syndrome is rare. In this report, a rare case of a 48-year-old woman who had an impacted supernumerary fourth molar in the bilateral mandibular ramus is described. She presented with a swelling in the left cheek region. Radiographic examination revealed an impacted supernumerary tooth in the left mandibular ramus with pericoronal resorption of the bone, suggesting peripheral inflammation. She also had an impacted supernumerary tooth on the right side. After administering an antibiotic and antiinflammatory drug, tooth extraction was performed under general anesthesia.
